Please complete the following Architectural Control Committee (ACC) request form. Please attach a professional diagram of the intended improvement in detail including materials, height, placement of improvement, accompanying equipment (if applicable) and color sample. When this application is complete, please send all information to Crawford Farms ACC, c/o Premier Communities Management Company, 2711 N. Haskell #2650, Dallas, Texas 75204 or fax to (214) 871-0355. This request will then be submitted to Crawford Farms ACC committee. You will receive a written response to your request within sixty (60) days of receipt of your request from the ACC.
